Is it Possible for a Lun to become Unlabeled, and remove itself from both metadata controllers' config files?

I mean, can this happen without human intervention?

Trying to figure out it someone tampered with something here...

XSAN 1.4.2

In Xsan 1.X (not 2.X) there can be a sudden relabeling of LUNs when using the Xsan Admin application to do something quite unrelated, such as adding or authenticating a client. This doesn't modify the .cfg file of the volume, but it requires very painstaking re-labeling of the LUNs back to their original names (with the volume stopped, of course) to get the volume back up without harm. It is a rare occurrence, but I have seen this phenomenon first-hand on more than one occasion.

I had it happen back in the 1.1 days. Be VERY careful, a wrong step will annihilate your filesystem. AppleCare should have a proper fix procedure by now (I was one of the first victims of this phenomenon and got to beta one of their early ineffective solutions- guess what happened to my filesystem).
